Integra Health Centre is currently accepting patients to join our Family Practice. Please add yourself to the waitlist and you will be contacted via email shortly.

NEW PATIENTS & INTAKE FORMS

At your first visit to Integra Health Centre and select subsequent visits (Physicals, New Patient appointments, well baby visits etc) you will receive an intake form for your appointment. This will be sent to your email and does not require printing. We ask that you complete this form prior to attending your appointment.

COVID-19 SCREENING QUESTIONNAIRE

In order to keep our patients, staff and the public safe we require a screening questionnaire to be completed a maximum of two (2) hours prior to your scheduled appointment. This will be sent to your email and can be completed on your electronic device.

MASKS

Integra Health Centre requires every patient and team member to be wearing a mask while in the clinic. Please ensure you bring one to your appointment.

APPOINTMENTS

All appointments at Integra Health Centre are conducted on an appointment-only basis, including our walk-in medical appointments.

Appointments can be made over the phone by calling 416-777-9333/9355, by emailing [email protected] and online.

For all scheduled appointments we reserve the time specifically for you. In order to respect your time, our Physician’s time and other patients’ time we ask that should you need to cancel or reschedule an appointment you do so a minimum of 24 hours prior to the start of your appointment. If you fail to cancel your appointment within the allotted time frame (unless an emergency arises) you will be subject to a cancellation fee depending on the nature of your appointment.

For appointments booked on the same day, it will be subject to a cancellation fee should you cancel or not show up for your appointment. If you are running late please ensure that you notify us as we have a discretionary grace period.

CONFIDENTIALITY/PRIVACY POLICY

Integra Health Centre takes the confidentiality of our patients’ health information extremely seriously and is committed to protecting the privacy and security of your personal information. Information collected by Integra Health Centre is used to provide you with healthcare services or to provide you with information regarding those services. Integra Health Centre does not sell your personal information for any purpose whatsoever. Integra Health Centre stores your records electronically through a secure portal. Your information is only accessible by our administrative staff and health practitioners that require access to this information. Any third party that requires access to your health records will only receive it with your consent. However, should we be required by law, we will comply with statutes that require us to disclose information to officials if required as part of an investigation. By using our services at Integra Health Centre you are consenting to the collection and use of personal and health related information set out in this policy.

EMAIL CORRESPONDENCE

Email should be used to schedule appointments or to communicate with the office for administrative issues only and should not be used for emergency cases. Any medical or clinical concerns should be addressed in person with your practitioner directly unless the practitioner has advised differently. Please be aware that email is not considered secure and Integra cannot be held responsible if there is any security breach or lost information during correspondence with our offices.

INSURED SERVICES (OHIP COVERED SERVICES)

You are required to present a valid OHIP health card at each visit.
In order to reduce health care fraud, no photocopies of health cards or written numbers can be accepted. Unfortunately, we cannot rely on previously documented health card numbers on your chart, since health cards expire or become invalid for multiple reasons.
You need to renew your green/picture OHIP health card if any of the following criteria are true:

  • your old card has expired.
  • you have moved since receiving your current card.
  • your old card has been reported lost or stolen.
  • your card is damaged or the magnetic stripe is unreadable.

Please note that the red and white OHIP cards, which previously did not have an expiry date, are now also being called in for replacement under the Ministry of Health’s expanded health card validation program. If you have a red and white OHIP card, and have received notice to seek a replacement card, you must do so before the indicated date, as your previous red and white OHIP card will no longer be valid.
If your current health card is not presented, is expired, or invalid, you will be billed directly for this appointment. You may still keep your appointment by paying $90 (uninsured OHIP fee) prior to your appointment. In order to receive a refund for services rendered, you must either call us or return to the clinic within 1 month of your visit with a valid and renewed health card. Please note that you must return with the original receipt and proof of payment in order to receive your refund. No exceptions will be made without your proof of payment.
Residents of other provinces (except Quebec) can be seen if they present a valid health card from the province of issue at the time of their visit. Without the physical card, a $90 fee will be charged.
If you are not covered under any Canadian Healthcare, you must pay a $90 fee for all medical visits. Official receipts are provided for insurance purposes. Please note that we are NOT a UHIP covered clinic.

TEST RESULTS

Most test results will be received by our office within 7 business days after being performed. They arrive directly to the physician who ordered them. Patients will receive a phone call if the results are in and the ordering physician would like to discuss with them. If you do not hear from our office; in most cases that means the doctor does not require a follow up appointment. If you would like a copy of your results or would like to discuss them please feel free to schedule an appointment with the ordering physician. Due to privacy reasons test results are never given over the phone.

PRESCRIPTION REFILLS

If you have run out of your medication and require a refill, please contact our office to book an appointment with a Physician to get this refilled. In some cases the pharmacy may request the Physician for a refill for you and depending on the situation the Physician may fax this back to the pharmacy requesting (please note this cannot be guaranteed and is dependent on both the Physician’s discretion and availability).

NON-INSURED SERVICES (SERVICES NOT COVERED BY OHIP)

Some of our medical services (notes, forms, travel consult’s, employment physicals, TB tests etc) are not covered by OHIP. You will be required to pay for these services at the time they are rendered and will be provided a receipt for your records.

 
 
 

Allied Health Patient Policy

APPOINTMENT’S

Appointments at Integra Health Centre are conducted on a one on one appointment basis. Prior to your first appointment with a new practitioner, you will be emailed an intake form along with consent forms to complete. These can all be done online through your electronic device. If you would like to complete this in office, please arrive 10 minutes prior to the start of your appointment.

For your convenience appointments can be made over the phone by calling 416-777-9333/9355, by emailing [email protected] and online through our secure booking portal.

When arriving for your appointment, please bring appropriate clothing to be able to freely move around should your treatment require this.

CANCELLATION POLICY

Your appointment time is held for you. Please understand that a missed appointment or late cancellation disables another client from accessing our services. As a result, cancellations less than 24 hours (unless an emergency), missed appointments or no-shows will be subject to the full session fee. Please be advised that our administrative staff will apply the fee according to the above criteria. Any issues or concerns regarding payment of the cancellation fee should be directed to management by emailing [email protected]

Appointments booked on the same day will be subject to a cancellation fee once the booking has been confirmed.

BILLING AND PAYMENT POLICY

Integra Health Centre does not provide direct billing for services rendered. At the time of your first appointment you will be asked to leave a credit card on file that will be charged after your appointment and for any cancellations or missed appointments. Your credit card is stored in a secure portal and cannot be accessed by anyone once stored and will only be used for billing purposes. Once services have been rendered and paid for you will be emailed a receipt with all the information required should you need to claim the session back through your insurance provider.
